Care Coordinator -Admin Ambulatory

Summary Position Summary The Care Coordinator will guide consumers and their families through the care continuum, establishing contact upon referral to the CCBHC and continuing contact throughout care and through a safe transition to primary care and/or self-management.
They will understand the clinical pathways established by the MBH CCBHC and direct the consumers to behavioral health services within the organization, at outside facilities, and within the community.
They will actively identify and address barriers to care that might keep the consumer from receiving timely and appropriate treatment.
The Care Coordinator will connect consumers with resources, health care and support services in their communities and assist the consumer in the transition from active treatment to graduation to primary care and self-care.
To accomplish this, they will work closely with the MBH CCBHC service staff to ensure the coordination and flow between services and providers of care.
The Care Coordinator will be responsible to ensure that consumer data is being entered and gathered correctly to meet CCBHC data reporting needs, including completion of the NOMS.
Required Minimum Knowledge, Skills, and Abilities (KSAs) Education:
Bachelor in social work or related field preferred License/Certifications:
None required
Experience:
Minimum of two years' experience working with behavioral health consumers and families.
Experience working in team based settings and collaborating with multiple providers preferred.
Demonstrated competency in applying evidence based knowledge, theory, practices, principles, and processes Compassionate, articulate and patient.
Knowledgeable about the breadth and accessibility of community resources.
Strong communication, interpersonal and organizational skills.
Ability to build and maintain solid interdepartmental relationships.
Strong problem solving, critical thinking, and advocacy skills.
Abillity to engage with external stakeholders, participate in community coalitions, and partner with police/corrections if needed Ability to identify potential crisis situations involving patients and/or families and to initiate appropriate action.
Recommended Skills Behavioral Medicine Clinical Works Communication Coordinating Critical Thinking Primary Care Estimated Salary: $20 to $28 per hour based on qualifications.
