Description:
Spurwink Services is looking for an organized, family-focused and advocacy-oriented individual to join our Children's Targeted Case Management team in our Biddeford office. We are seeking individuals who are highly organized, have good time-management skills, appreciation for ongoing professional learning, and computer competency. Case managers will be required to have excellent communication/engagement skills with families and colleagues. Case Managers assess needs, develop service plans, make referrals and monitor outcomes with children and families, their natural supports, and with other committed professionals who serve to reduce barriers to families. Use of own car (with mileage reimbursement) is necessary. Occasional evening hours are required.
Duties:
Provide intake/assessment when clients are referred. Facilitate Individual Service Plan team meetings and develop a plan of care (ISP) for the child with input from the team. Plan will include a crisis plan. Oversee implementation of ISP, monitor and document progress. Maintain regular contact with team members and other community professionals involved with child. Ensure that clients and families are aware of all community services and resources. Assist families in accessing resources. Provide family support through phone contact and face-to-face meetings, home visits and community outreach. Participate in individual & group supervision. Know and comply with policies and procedures. Maintain records and documentation as required by Spurwink or State Agencies. Maintain Productivity Expectations. Monitor Mainecare Authorizations. Maintain a current Maine driver's license and a clean driving record as outlined in Spurwink's Driving Policy.
Qualifications:
Bachelor's Degree in Social Services or a related field. Experience in Social Services preferred.
